Volunteer Forms

We at VME are incredibly grateful for the many hundreds of volunteer hours we receive from families and friends each year. For many of these volunteer activities, certain forms and clearances are required. As we strive to continue to keep our children safe, we have provided access to the following forms and clearances. If you are asked to provide any of these clearances to VME, we will keep them on file - you will not have to provide a new set of forms each time you take on a new project.

If you have any questions or need a paper copy of any of these forms, please contact the main office at the school.
